To assist Princes Hill Secondary College in assessing your child’s eligibility for enrolment and facilitate a smooth transition into Year 7, the following information and documents should be provided. Please review all the guidelines provided below and prepare documents PRIOR to commencing the online form.

Information Required

Click on each heading below to see guidance on information that must be submitted using the Transition Enrolment Information Form at the bottom of this page.

At Princes Hill Secondary College students are required to study a Language other than English (LOTE) through Year 7 and 8 with French or Italian being offered. Classes and Home Groups for Year 7 are allocated based on this preference. It is important that families give careful consideration as to which LOTE is preferred as this will directly influence class allocations and the preference submitted will be considered final for the purpose of class allocations. Options provided are:

  • French (studied previously)
  • French
  • Italian (studied previously)
  • Italian
  • Don't mind

An additional family is a family or parent the student lives with some of the time, or who they have regular contact with in addition to the family at the nominated primary household (the address used for application). Some examples of additional families may include, but are not limited to, a biological parent and a step parent, a biological parent living alone, grandparents, a case worker, a family friend, a foster parent or other relative. Princes Hill Secondary College would like to know if the student has an additional family so contact and other information can be confirmed.

Princes Hill Secondary College acknowledges the importance of volunteers and recognises the valuable contribution that volunteers provide to our school community and the work that we do. Please review our Volunteers Policy to gain an understanding of how the school ensures that volunteers are suitable to work with children and are well-placed to make a positive contribution to our school community. We would like to understand your interest in volunteering to assist in a variety of activities, including:

  • in our classrooms
  • with sports days and events
  • school camps
  • working bees
  • excursions and incursions
  • school concerts

We are seeking your interest to be informed and included in opportunities to volunteer in these activities; it does not represent a commitment to any or all of those activities listed above.

Input provided should be for the student primary household; additional households will have an opportunity to specify volunteer preferences at a later point when we confirm enrolment details for the student. The nomination of Parent A and Parent B is relatively arbitrary but should align with what has been specified as part of the enrolment details provided to your primary school. Options are to select none, one or both of Parent A and Parent B.

Princes Hill offers a robust music program that covers instrumental music lessons, music theory lessons, ensembles, and classroom music. Please take the time to navigate our Music Department Home Page to see the various resources we have available and to find out more about our program.

Due to the high demand for the program we have at times had to manage wait lists on some instruments. To assist with the enrolment process you are invited to express interest in your child engaging in the many aspects of our music program and additional enrolment information will be forwarded to those that express interest.

Prepare Documents for Upload

There are two documents that must be provided when submitting the Transition Enrolment Information Form at the bottom of this page.

Families are requested to provide documentation that verifies the address of permanent residence for the student used in the application process. Please see the Department of Education Residential Address Check document for a definition of permanent residence and a listing of valid documents that may be submitted as part of a 100-point residential address check.

One of the following documents MUST be uploaded:

  • A current Council Rates Notice, OR
  • A current Lease Agreement, OR
  • Exchanged Contract of Sale, OR
  • A current utility (Electricity, Gas, Telephone or Internet) bill showing the service address

This will be matched to the information we receive from the primary school. On request, you may also be required to complete a statutory declaration confirming the student is living at the address provided in the enrolment application and that the arrangement is genuine and intended to be permanent.

Important

Address Verification Documents are NOT retained by the school and will be destroyed once address verification is complete, in compliance with the Department of Education Privacy Policy and Privacy Collection Notice.

Please prepare a scanned file of the student birth certificate as proof of legal name and date of birth. Where a birth certificate cannot be produced, other acceptable evidence of a student’s full name and date of birth includes a passport, citizenship documents or Australian visa documents.

The image file should be in PDF format and named as follows:  <Document Type> <SURNAME> <First Legal Name>.pdf   - for example, Birth Certificate BLOGGS Morgan.pdf or  Passport BLOGGS Morgan.pdf

The birth certificate (legal name and date of birth) image file will be retained as part of the student record adhering with the Department of Education Privacy Policy and Privacy Collection Notice.

There are a number of additional documents that families can submit to support the enrolment process. These may not apply for all students, but will be required where applicable and families are encouraged to use this opportunity to upload the relevant documents.

Any information or management plans that will assist us in management of medical/health issues such as anaphylaxis, asthma etc. Princes Hill Secondary College expects parents to provide medical management plans and medications for their child as required. See the College Medication Policy for information and forms.

Parents, carers, students and school staff are expected to provide the principal or a member of the school leadership team with relevant information and documentation about any family violence intervention orders or personal safety intervention orders, that may impact on students or staff or the way in which the school manages its operations.

School staff manage information relating to intervention orders sensitively and in accordance with privacy laws. You may either upload current parenting plans or intervention orders as part of this enrolment process or provide to the school subsequently.

During the school year there are many occasions and events where staff may photograph, film or record students participating in school activities and events. We do this for many reasons including celebrate student participation and achievement, showcase particular learning programs, document a student’s learning journey/camps/excursions/sports events etc.  The school asks that any parents/carers or other members of our school community photographing, filming or recording students at school events such as music performances, public events, etc, do so in a respectful and safe manner and that any photos, video or recordings (“images”) of students are not publicly posted (eg to a social media account) without the permission of the relevant parent/carer.

Princes Hill Secondary College understands that parents and carers have the right to withhold permission for our school to use photographs, video or recordings of your child (apart from circumstances where the school is not required to seek consent – see our Photographing, Filming and Recording Students Policy).

Please read the attached Annual Consent Form and Collection Notice that applies to photographs, video or recordings of students that are collected, used and disclosed by the school.

If you have read this notice and are comfortable with the school using photos, video or recordings of your child as described above, you do not need to take any further action.

However, if you have decided that you do not want images of your child to be collected or used by our school, please fill in the Annual Consent Form and Collection Notice, scan to a PDF format and upload. Alternatively, you may deliver the completed consent form to the school.

If you child is attending a Victorian Government Primary School, enrolment information will be automatically transferred from the primary school to Princes Hill Secondary College. Once received, we will be forwarding a copy of that information to families for checking and confirmation of accuracy.

However, if your child is coming from a non-Government Primary School or an international/interstate school, then you will be required to lodge the standard Department of Education Student Enrolment Form. Please download this form and fill in (electronically is preferable), and then save/scan the document in PDF format for upload.
