This page allows the secure upload of a Transition Student Birth Certificate image. Prior to clicking on the link below to upload documents, please prepare a scanned image file of the student Birth Certificate as proof of legal name and date of birth. Where a birth certificate cannot be produced, other acceptable evidence of a student’s full name and date of birth includes a passport, citizenship documents or Australian visa documents.

The image file should be in PDF format and named as follows:  <Document Type> <SURNAME> <First Name>.pdf   - for example, Birth Certificate BLOGGS Morgan.pdf or  Passport BLOGGS Morgan.pdf
