To assist Princes Hill Secondary College in assessing your child’s eligibility for enrolment, documentation is required to verify the address of permanent residence. Please see the Department of Education Residential Address Check document for a definition of permanent residence and a listing of valid documents that may be submitted as part of the 100-point residential address check. In addition to the address verification documents, a Birth Certificate (or equivalent documents) must be provided in order to verify the child's legal name and date of birth.

Prepare Address Verification Documents for Upload

Address Verification Documents: It is requested that all address verification documents that add up to at least 100 points are scanned (PDF format preferred). Address related documents may be combined into a single file or uploaded separately, however, the address documentation should not be combined into a file with the Birth Certificate documentation.Birth Certificate: Please prepare a scanned image file of the student birth certificate as proof of legal name and date of birth. Where a birth certificate cannot be produced, other acceptable evidence of a student’s full name and date of birth includes a passport, citizenship documents or Australian visa documents. The image file should be in PDF format and named as follows:  <Document Type> <SURNAME> <First Name>.pdf   - for example, Birth Certificate BLOGGS Morgan.pdf or Passport BLOGGS Morgan.pdf.

Once documentation has been prepared, please use the links link below to upload . It is important that two separate uploads are performed, as address verification documents are the Address Verification Documents. Note that Address Verification Documents are NOT retained by the school and will be destroyed once address verification is complete. However, the birth certificate (legal name and date of birth) image file is retained as part of the student record adhering , in compliance with the Department of Education Privacy Policy and Privacy Collection Notice.
