Dear Parent/Guardian/Carer,

The Department of Education and Training requires that all Victorian Public Schools maintain accurate records for students in the CASES21 Student Information Database (see Enrolment Policy). This information is important for the smooth operation of the school but also plays a critical role in the Student Enrolment Census, which is used by the Department for funding allocation, provisioning, planning and national reporting.

This page provides the ability for the Parent/Guardian/Carer of a student at Princes Hill Secondary College to submit an Enrolment Form that will be used to confirm/update student details on the CASES21 Student Information Database. We recognise that there may be different preferences for submitting the Enrolment Form and we offer a number of options below - please select a single option only and complete all steps for that option. The preferred option is the first one where you enter all enrolment information using our Online Enrolment Form; it provides guidance on each of the questions and allows some automation in the checking of your enrolment data.

Note all details submitted are treated as confidential and handling of the information will be done in compliance with the Department School's Privacy Policy.


IMPORTANT

The form requests some information that is very important to the funding and operation of the school and it is requested you take particular care to provide accurate details in relation to:

  • Student country of birth and residency status
  • Home language spoken
  • Aboriginal/Torres Strait Islander status
  • Health and medical information
  • Parent occupation, education, country of birth and language spoken

Please check the Parent Occupation Group instructions (and the A-Z Occupation Listing) prior to commencing the enrolment form to ensure that your current occupation and the associated group code are entered accurately.

Simply mark-up enrolment details on the form, scan or save an electronic copy of the form and then use the link below to upload an electronic version of the form (PDF is the preferred format).

Option 3 - Hand in Enrolment Form at the Front Office

This is the least preferred option, but families may print the PHSC Enrolment Form, fill in the details and then simply drop the completed paperwork at the Front Office.
